Listed below is information we request prior to admission to one of our facilities. Please fax the following information/documents to the Housing Department at 931-507-1218.

General Information (including SSN, DOB, address, etc.)
Insurance Information (copies of all insurance cards, if applicable)
Income SSI/SSD Information
Present amount of financial resources (please explain)
Psychiatric Evaluation
History and Physical
Current Medication List

1. 3-day supply of medications OR
2. Prescriptions prescribed and/or filled prior to member arrival

Progress Notes
AIMS Assessment
TB skin test results OR chest x-ray results
Hepatitis C Panel
Current Labs
Pregnancy Test
