Care Manager
Location: Generations Mental Health Center
Dresden, TN – Full Time
Qualifications:
Bachelor’s degree in health related field of counseling, psychology, social work, sociology, behavioral science field
Position Summary:
The Care Manager provides assessments to determine potential strengths, resources and basic needs through formal and informal evaluations. These assessments include assessment activities, service planning, daily functioning, and referral/coordination of care, crisis facilitation and liaison activities. These assessments will be conducted in coordination with the member, member’s family/caregiver, residential home Executive Director and Activities Directors
